Please note that we currently have a low volume of computers available and that it may take additional time to provide you with a computer. If you have
any questions/comments/concerns please use the contact us page or include a note with your application.
After your application has been recieved and processed you will be contacted with the result. If computers are available please allow at least 4 - 6 weeks for processing. Since all of are computers come from donations there may be a significant wait if no computers are available. If you have not been contacted then no computer is ready for you and you will not be able to pick one up.
All desktop computers have a $50 administrative fee to cover refurbishment costs (including the installation of Genuine Microsoft Software).

How to fill this form out electronically

Download the application pdf from the above link by clicking on it. This should open the application in your internet browser. If you don't have Adobe Reader, follow the link below the application to download and install it.

After reviewing the application process, you may begin filling out the application form directly (do not worry about signing it).

After you have completed filling out the form, click on the save button.
This will bring up the save dialog:
Type in the name of the form, anything that you will remember will work fine, and choose where you want to save it. I would recommend saving to your desktop (as shown in the preceding image). Click on the save button when finished. This will save the application with what you have typed into it onto your computer.

Now that you have the application pdf saved on your computer you can attach it to an email and send it to:
